The Family Access level membership is discounted Family membership option for those who participate in the following programs: CareSource, Ohio Directions card, Medicaid card, W.I.C. card, Molina, SNAP and EBT. When purchasing a Family Access membership online, you must agree to verify your participation in a qualifying program during your next visit upon check-in by showing documented proof, such as a card. At that time, you may request a physical or digital Conservatory membership card and begin using your member benefits.
